LVN Skilled Nursing

The Licensed Vocational Nurse (LVN), reporting to the Director of Nursing, provides direct and individualized nursing care to residents in a Skilled Nursing Facility (SNF). The LVN collaborates with physicians, nursing staff, residents, families, and interdisciplinary teams to ensure safe, high-quality, patient-centered care.

Location: Alturas, CA 96101

Duration: 13 weeks contract with possible extension

Shift: Days, Nights, Weekends

Guarantee Hours: 36

Scrub Requirement: Caribbean Blue scrubs

Pay rate: $45- $50/hr.

Qualifications:

• Active CA LVN license
• Current BLS/CPR certification
• SNF, geriatric, or rehab experience preferred
• Physically and mentally able to perform nursing duties, including lifting up to 35 lbs
• Strong assessment, communication, and documentation skills
• Reliable, adaptable, and team-oriented

Key Responsibilities:

• Administer medications and treatments per physician orders and facility policy
• Monitor, assess, and document resident conditions and changes
• Supervise and support nursing assistants; ensure assigned care is completed
• Assist with care plans, admissions, discharges, and resident education
• Respond to emergencies and communicate promptly with physicians and families
• Maintain infection control, safety standards, and regulatory compliance
• Participate in staff education, meetings, and required in-services

Work Conditions:

Full-time position; 812 hour shifts as scheduled

May require flexibility in hours and assignments
