As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative team. This diverse team includes physicians, pharmacists, care coaches, behavioral health specialists, and more. Together, they deliver personalized, high-quality care.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience providing value-based care.

What We‘re Looking For

Required:

• You will have a Master‘s Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, we will require you to undergo TB screening.
• Spanish speaking

Preferred:

• You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers

Additional Information:

• Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The Federal Government will require applicants to take the Interagency Language Rating (ILR) test.
• You‘ll work onsite at the State Ave. clinic
• You‘ll report to the Area Medical Director

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
