Responsibilities
- Evaluate and treat patients using medically prescribed physical therapy programs to restore function and prevent disability.
- Develop, implement, and modify plans of care; document observations, clinical findings, problems, interventions, goals, and discharge plans.
- Conduct initial admissions/evaluation visits and monitor patient progress toward functional goals.
- May supervise Physical Therapy Assistants as appropriate and within scope of practice.
- Notify the Clinical Team Manager of OASIS scores and other required documentation within established timelines.
- Collaborate with the interdisciplinary team to optimize patient outcomes and coordinate discharge planning.
- Provide patient and family education on home exercise programs, safety, and activity modification.

Requirements
- One year of home health experience
- Ability to work in a home-based care setting and travel within the service area as needed
- Strong documentation skills and ability to maintain accurate patient records
- Effective communication and collaboration with patients, families, and the care team
- Current eligibility to practice as a Physical Therapist in the United States; specific licensure details not provided in posting
