Job Summary Critical Access Hospital. Primarily outpatient, but will also cover some inpatient and SNF. Key Responsibilities Provide outpatient physical therapy services Cover inpatient physical therapy needs as required Provide skilled nursing facility (SNF) physical therapy services as needed Required Skills Outpatient experience Acute Care experience LTC/SNF experience Education Requirements PT License required BLS certification required Pending License accepted Required Experience Minimum 1 year of experience First-time therapists accepted Work Environment / Physical Requirements No floating required within scope to meet facility needs No on-call requirements Block schedule accommodations available Guaranteed 70 hours per 2 weeks; traveler can be called off for 1, 10-hour shift per 2 weeks Facility provides housing (1-bedroom apartments typically); housing available no earlier than 72 hours before contract begins and no later than 24 hours after contract ends Pets are not allowed in housing Housing units typically accommodate only the traveler; additional family members require facility approval Housing unit must be cleaned upon departure or additional cleaning fees will be billed to traveler Facility provides a shared-use rental vehicle for traveler use around town within a 30-mile radius from facility Vehicle available no earlier than 72 hours before contract begins and no later than 24 hours after contract ends Only the traveler is authorized to operate the vehicle Traveler must provide copy of valid driver's license and proof of insurance prior to operating vehicle Traveler's personal vehicle insurance policy must have rental car insurance to cover accidents/damages Vehicle must not be taken off-road or outside 30-mile radius from facility If traveler has time off, vehicle must be turned in during time off and a new vehicle will be issued upon return Tire chains available at additional cost to traveler Vehicle must be cleaned and returned with full tank of gas Important Notes Candidates must have permanent address at least 50 miles from facility (locals not accepted) If candidates have worked as permanent staff at this facility, they must be separated for at least 3 months prior to working as a traveler Time off approved on an individual basis upon offer Benefits Medical, Vision, and Dental Insurance Plans 401k Retirement Fund #J-18808-Ljbffr United Cerebral Palsy of Georgia
