Job Title: Home Health RN Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience License: Active RN License Certifications: CPR Certification required Must-Have: - 1 year Case Management Experience - Ability to travel to assigned cases - Weekend rotations required once every 6 weeks Description: As a Home Health RN, you will be responsible for assessing patient conditions. You will implement care plans tailored to individual patient needs. Providing ongoing education and support to patients and their families is essential. Patient care must align with organizational philosophy, policies, and community health standards. You will perform admission processes and manage cases in accordance with established policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications is required.
