HIV Case Manager - Integrated Care Advocate

Parkland Health's ACCESS Clinic in Dallas seeks a Behavioral Health Case Manager to establish and maintain an integrated behavioral health process for HIV primary care under Ryan White funding. You will develop, implement, and monitor individual care plans and coordinate with patients and care teams to ensure access and quality services. The ideal candidate has a Master’s in Social Work or nursing, at least four years of hospital or community-based patient care, and licensure or certification as #J-18808-Ljbffr Parkland Health
