Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness for physical therapy.
- Document and complete a Plan of Care (POC), progress notes, and other clinical records. Revise the POC based on ongoing assessment data.
- Plan and provide treatment for various disorders based on medical referrals and therapy assessments.
- Assist and instruct patients and families on proper transfer techniques and the use of supportive equipment.
- Educate patients and families regarding therapeutic procedures tailored to the patient's condition and needs.
- Ensure the application of infection control and universal precautions in patient care activities.

Compliance / Notes
- Must have commitment to meet continuing education requirements.
- Requires the ability to work under stress to meet deadlines and patient needs.
