MDS Coordinator

We are currently seeking a MDS Coordinator professional to join our team. $2,500 sign-on bonus available Responsibilities: Oversee and facilitate the completion of resident assessments in accordance with Federal and State regulations Establish assessment schedules and ensure that assessments are completed accurately and in a timely manner Transmit assessments to the Massachusetts Department of Health Coordinate resident care plans in accordance with regulatory requirements, ensuring effective delivery of care Initiate Medicare coverage for newly qualified residents and issue denial letters when necessary Stay up to date on changes in Medicare coverage and determine required documentation for reimbursement Coordinate the assessment, planning, implementation, and evaluation of resident care through the MDS process Facilitate care conferences with the interdisciplinary team Coordinate, complete, and review Minimum Data Sets (MDS) Educate, train, and support nursing staff and other departments on assessments, care planning, and the MDS process Ensure all current MDS updates are implemented and staff are properly in-serviced Conduct regular resident interviews as required by MDS Requirements: Must be a licensed Massachusetts RN or LPN Minimum 1 year of experience as an MDS Coordinator Strong communication, organizational, and assessment skills #J-18808-Ljbffr
