Shift: 12H Days 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday

Contract Length: 13 weeks

Job Description:

The Social Worker will be responsible for acute inpatient medical case management, including discharge planning and managing a caseload of complex patients at a Level 1 Trauma Center.

Responsibilities:

Provide acute inpatient medical case management and discharge planning for a diverse patient population.
Collaborate with interdisciplinary teams, including Respiratory Therapy and Pharmacy, for discharge planning.
Manage a caseload of 15-20 patients per day, ensuring timely and effective discharge processes.

Requirements:

Active MS State License as LMSW or LCSW.
Minimum 2 years of acute inpatient medical case management experience, with at least 1 year in a large hospital setting (500+ beds, Level 1/Academic Medical Center).
Strong computer skills and proficiency in EPIC charting.
Recent acute care/ED discharge planning experience within the last two years.
BLS Certification required.
