Assignment Details:
- Guaranteed Hours: 36
- Contract Length: 13 weeks
Requirements:
- VT licensure
- Valid driver's license
- Reliable vehicle
- Experience in a home care setting preferred
Roles & Responsibilities:
- Provide in-home therapy services to enhance patient independence and safety.
- Collaborate effectively as part of a multidisciplinary team.
- Conduct assessments and develop individualized treatment plans.
- Monitor patient progress and adjust interventions as needed.
- Educate patients and caregivers on therapeutic techniques and safety measures.
- Maintain accurate and timely documentation of patient care and treatment.
