Resumen Los Planes y Redes de Banner (BPN) son una red integrada para Medicare y planes de salud privados. Como Navegador de Atención Comunitaria, estarás conectando las oficinas de PCP de los pacientes, los Gerentes de Casos y la atención hospitalaria. Trabajarás en el Centro Médico Banner Thunderbird, el Centro Médico Banner Baywood, el Centro Médico Banner Casa Grande, el Centro Médico Banner Boswell o el Centro Médico Banner Del Webb. Tu papel implica iniciar conversaciones, construir relaciones y colaborar con los proveedores de atención médica. Responsabilidades Iniciar conversaciones, construir relaciones y colaborar con los proveedores de atención médica. Referencias o asignación a otros departamentos, coordinación de atención de salud a domicilio, confirmación de arreglos, citas con médicos o ambulatorios. Asistir con la navegación del paciente a través del sistema de atención médica conectando a los miembros con información o servicios. Educar a los miembros internos del equipo de atención médica sobre la gestión del cuidado, conceptos de atención administrada y recursos comunitarios. Priorizar eficazmente y tomar decisiones sensatas siguiendo las políticas, procedimientos y estándares establecidos por el departamento. Realizar múltiples tareas en un entorno de ritmo rápido con interrupciones frecuentes. Trabajar de manera cohesiva en un entorno de equipo con múltiples disciplinas. Calificaciones Preferidas Trabajador de Salud Comunitaria, Asistente Médico u otra certificación. Un año de experiencia con Medicaid, Seguro Comercial y/o Medicare. Terminología médica y/o fondo.
