Hospice Physical Therapist - Home Care Specialist

Optum Wythe Circle Home Care in Wytheville, VA, is seeking a licensed Physical Therapist to provide hospice services, including comprehensive assessment, direct treatment, home exercise program development, patient/family instruction, and documentation according to hospice standards. You will initiate evaluations per physician orders, document each visit, participate in the Interdisciplinary Group, educate the patient and family on functional goals, and prepare discharge summaries when care #J-18808-Ljbffr Optum
