Insight Global is seeking a Clinical Pharmacist to support Signature Healthcare in Brockton, MA. This is a 12-13 week contract opportunity created to provide coverage for a pharmacist who will be out on medical leave.\n \nThis position is a primarily administrative, desk-based clinical pharmacist role focused on medication management, treatment plan oversight, and regulatory compliance rather than direct patient care. The Clinical Pharmacist will work closely with providers and care teams to review medication therapies, evaluate clinical appropriateness, ensure compliance with established guidelines, and make recommendations that support safe and effective patient outcomes.\n \nResponsibilities:\nReview and evaluate medication therapies for appropriateness, safety, efficacy, and compliance with clinical guidelines\nAnalyze treatment plans and provide recommendations to physicians and care teams\nReview medication dosing for patients ranging from neonates through geriatrics\nMonitor and manage anticoagulation therapies, including medication selection, dosing considerations, and monitoring parameters\nReview medication reconciliations and identify potential medication-related issues\nAssess drug interactions, contraindications, adverse reactions, compatibilities, and therapeutic outcomes\nConduct clinical reviews using pharmacy literature, research, and reference resources\nSupport adverse drug reaction monitoring and reporting initiatives
