Duties

• Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene.
• Monitor patients’ health status by observing vital signs and overall condition, reporting any changes promptly.
• Support medication administration following care plans and HIPAA regulations to ensure privacy and safety.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption if necessary.
• Facilitate patient transfers using proper manual handling techniques and equipment such as Hoyer lifts or patient transfer methods.
• Maintain a clean and safe environment through regular cleaning, sanitation, and organization of client spaces.
• Provide transportation assistance for appointments, errands, or community outings to promote independence and engagement.

Requirements

• Spanish Speaking
• Knowledge of patient observation techniques, dementia care (including Alzheimer’s care), memory care, and hospice care practices.
• Ability to perform heavy lifting safely and assist with manual handling tasks using proper patient transfer methods
• Strong communication skills for effective client interaction and documentation of care plans or progress notes.
• Valid certifications in first aid and CPR
