PRN LPN Opportunity

We are seeking a PRN LPN to provide nursing support to clients within our agency. This role is critical to meeting current care plan requirements, skilled nursing in a home setting. The Licensed practical nurse performs assessments and simple medical nursing procedures in client's homes, under the guidance of the RN Supervisor, in accordance with physician's orders.

Hours: self-scheduled during the week. Monday through Friday.

Responsibilities and Duties Include:

• Medication set-ups and reviews
• Skin assessments and general health evaluations
• Basic nail care
• Client education and documentation
• Enable patients to stay independent while they remain in their own homes.
• Perform routine supervisory visits
• Follow up for unskilled services
• Maintain a high degree of confidentiality at all times due to access to sensitive information.
• Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
• Follow all Medicaid and HIPAA regulations and requirements.
• Abide by all regulations, policies, procedures and standards.
• Performs other duties as assigned.

Position Requirements & Competencies:

• Graduation from an accredited school of nursing and current LPN licensure in the state of practice
• New Nurses welcome, will train!
• Ability to make home visits and provide nursing care in the home environment
• No weight restrictions
• Home Care experience preferred but not mandatory
• Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
• Ability to complete necessary documentation appropriately, legibly, and timely
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
