Access: Supports for Living in Middletown, NY is seeking a Housing Case Manager to support chronically homeless adults and families by developing a Housing Health and Recovery Plan centered on person-centered recovery. You will secure affordable housing, address life domains (employment, finances, health, relationships) and partner with recipients to navigate available supports while planning a thoughtful discharge when ready to transition from Supported Housing. #J-18808-Ljbffr Access: Supports for Living
