Join the Hamden Rehabilitation and Healthcare Center team and make a difference in the lives of our residents! As a Director of Social Services, you will be responsible for leading our social services department and ensuring that our residents receive compassionate and personalized care.

About the Role:

As a Director of Social Services, you will be responsible for:

• Leading and managing a team of social workers and social service coordinators to provide exceptional care and support to our residents

• Developing and implementing social services programs to meet the unique needs of our residents, including case management, counseling, and referrals

• Coordinating with other departments to ensure seamless communication and collaboration

• Maintaining accurate records and reports , and ensuring compliance with regulatory requirements

• Participating in quality improvement initiatives to continually improve our services and outcomes

Requirements:

• Master's degree in Social Work or related field (or equivalent experience)

• Minimum 1 years of experience in social services or a related field

• Strong leadership and management skills , with the ability to inspire and motivate a team

• Excellent communication and interpersonal skills , with the ability to work effectively with residents, families, and staff

• Ability to maintain confidentiality and handle sensitive information with discretion
