Overview
Physical Therapist needed for a travel assignment in a skilled nursing facility in Milford, New Hampshire, United States. Start date: 09/14/2026 for 13 weeks. Weekly pay: $2,481–$2,657 per week with guaranteed hours of 35. Shift is 8:00 AM to 4:30 PM, five days per week. This is a travel (temporary assignment) position through Healthforce Healthcare LLC.

Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Administer therapy sessions (e.g., therapeutic exercises, gait training, and modalities) and monitor patient progress.
- Document assessments, goals, treatment plans, and progress notes in a timely and compliant manner.
- Collaborate with an interdisciplinary team to support discharge planning and optimize resident outcomes.
- Educate residents and families on home exercise programs, safety, and continuity of care.

Requirements
- Current, active Physical Therapist license in New Hampshire or eligible for NH licensure.
- Minimum 1 year of physical therapy experience in a skilled nursing facility or rehabilitation setting preferred.
- Strong documentation, communication, and collaboration skills.
- Ability to work effectively in a skilled nursing facility environment and with an interdisciplinary team.
