Patient Care Technicians - Telehealth Patient Care Coordinator

Virtual Care Coordinator

As an integral part of a team, the Virtual Care Coordinator is responsible for supporting virtual patient care activities and promoting patient-focused care. This role involves performing various remote monitoring duties related to patient safety and clinical needs. The Coordinator provides support and assistance with telemedicine services. Duties include monitoring patient status and hospital environmental conditions. Collaboration with leadership is required for implementing telehealth and remote patient monitoring programs. The role includes training coordination and participation in the development of telehealth best practices. Clear communication with clinical teams, patients, and vendors is essential. Accountability for clinical quality and safety outcomes is shared.

Essential Duties and Responsibilities:

  • Performs quality assurance and peer reviews of the telehealth program.
  • Evaluates the virtual patient observation program in collaboration with leadership.
  • Makes recommendations for improvement based on findings.
  • Meets regularly with senior leadership to discuss quality metrics and workflow expansion.
  • Collaborates on case reviews and implements solutions.
  • Triages and assigns cases for virtual monitoring or observation.
  • Collaborates on telehealth policy development and implementation.
  • Maintains an operational database for telehealth program performance and quality metrics.
  • Identifies trends and participates in corrective action plan development.
  • Works with staff to efficiently utilize telehealth platforms.
  • Successfully completes Virtual Care Coordinator orientation.
  • Provides training on telehealth workflows and documentation requirements to new staff.
  • Acts as a liaison between physicians, clinical staff, and vendors.
  • Facilitates analysis of virtual patient care utilization reports and assesses eligibility for virtual care.
  • Coordinates transitions with hospital care teams based on patient criteria reports.
  • Navigates multiple technology platforms to ensure quality care.
  • Provides real-time support to troubleshoot telehealth platform issues.
  • Monitors assigned patients and observes for changes in clinical status or behavior.
  • Ensures safety and prevents falls and injuries as directed by the registered nurse.
  • Performs monitoring of clinical early warning signs.
  • Communicates and collaborates effectively with patients and clinical teams.
  • Reports any variations in patients' clinical status to the registered nurse or physician.
  • Documents patient behavior and interventions, escalating concerns to the clinical team.
  • Suggests to the registered nurse if a patient can be discharged from remote monitoring based on observations.
  • Monitors telehealth consult orders and scheduled visits to ensure timely sessions.
  • Greets patients and families, introducing self and role.
  • Answers calls from clinical teams promptly.
  • Adheres to approved communication methods with clinical teams.
  • Assists in interpreting as needed, using approved methods.
  • Ensures patient privacy by adhering to information security policies and procedures.
  • Escalates patient safety concerns to nursing leadership.
  • Reports issues with telehealth and remote monitoring equipment.
  • Maintains accurate records of interventions and safety events.
  • Demonstrates the ability to prioritize, problem-solve, and be flexible.
  • Communicates effectively in English, both orally and in writing.
  • Submits utilization and assignment reports every shift to leadership.
  • Attends staff meetings and keeps up to date on new initiatives.
  • Maintains attendance and punctuality records in compliance with policy.
  • Ensures a safe and clean working environment.
