Observation Unit Advanced Practice Provider Evening PD

Overview The Observation Unit Advanced Practice Provider Advanced Practice Provider utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN or PA works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The position will cover multiple cardiac service lines where needed (Observation, HF/MI, and Cath lab). Minimum of 3 years of experience in an acute care setting (hospital-based experience) required. Cardiology experience strongly preferred. Current APP licensure and certification (ACNP or PA) in good standing. Strong clinical assessment and decision-making skills. Excellent communication skills, proven team player, ability to multitask and prioritize, and proactive and creative problem-solver with a focus on process improvement, care coordination, and patient-centered solutions. Evenings, per diem Responsibilities Perform and document accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem. Order, perform, and interpret laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate. Manage general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and indications and contraindications of pharmacologic agents and other treatment modalities. Prescribe medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed. Provide medical services under the supervision of a licensed physician, consistent with the PA's education, training, and experience. Promote Hackensack Meridian Health (HMH) practice recognition through conferences, publications, and presentations. Provide patient- and family-centered, compassionate care that promotes health and treats disease. Practice as a healthcare provider according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. Apply evidence-based and value-based medicine to systematically analyze and improve patient care practices and outcomes. Evaluate patient/family transition of care planning with resource utilization, cost, quality, safety, and self-management promotion for chronic conditions. Demonstrate clinical expertise through advanced assessment and procedural skills pertinent to the practice specialty. Translate best practice into clinical practice by evaluating current protocols and standards of care. Provide culturally sensitive and educationally appropriate health management education to patients and families. Facilitate consistent, coordinated care and clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds. Serve as an advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate. Serve as a clinical resource for the multidisciplinary team, addressing clinical, process, and system improvement opportunities. Participate in organizational and departmental meetings, committees, and workgroups as necessary. Demonstrate the highest level of accountability for professional practice and adhere to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes. Promote a collaborative work environment supporting the organizational mission, behavior expectations, values, and pillars. Mentor other PAs or APNs, precept students in PA programs, and participate in educational programs as a clinical expert. Identify patient/family educational needs and collaborate to implement appropriate teaching materials. Promote innovation by integrating new clinical knowledge into practice. Participate in clinical research activities and disseminate findings through practice, education, presentations, and publications. Identify ethical care issues and coordinate interdisciplinary meetings to resolve them. Comply with HMH and department-specific scope of services, policies, and procedures as applicable to the role. Adhere to all HMH infection control and safety policies. Perform other duties and/or projects as assigned. Qualifications Education & Certifications Graduate of an NLN/AACN accredited nursing program. Master’s or Doctorate degree from an accredited Nurse Practitioner program. Graduate of an accredited Physician Assistant program accredited by the AMA Council on Medical Education. Current, unrestricted New Jersey Physician Assistant License. Current, unrestricted New Jersey Advance Practice Nurse License. Board certified by AANP or ANCC. Experience Maintains clinical competencies through continuing education and clinical practice. Experience in a relevant patient care setting (or as regulations dictate). Licensure & Certifications National Provider Identifier (NPI). Prescriptive authority through CDS and DEA registration according to New Jersey state law. Collaborative agreement with physicians on staff. Current Basic Life Support (BLS) certification from the American Heart Association (AHA). Advanced Cardiac Life Support (ACLS) certification (preferred). Skills & Abilities Excellent written and verbal communication skills. Proficient computer skills that include but may be limited to Microsoft Office and/or Google Suite platforms.
