Description
Join our team as a dedicated professional in a foster care program for children with special medical needs.
This full-time onsite position in Manhattan, NY, involves collaboration with nurses and social workers to provide intensive casework services.
The role requires excellent communication skills and the ability to work under pressure.

Requirements
Work collaboratively with a team of professionals including nurses and social workers.
Conduct intensive outreach to connect children and families with resources.
Maintain comprehensive written case records.
Partner with assigned RN for home visits.
Train foster parents in specialized child care skills.
Perform other duties as assigned.

Qualifications
Bachelor's Degree in Social Work or related field required.
Foster care experience preferred.
Bilingual (Spanish/English) preferred.
Strong verbal, writing, and organizational skills.
Flexibility and ability to meet deadlines under pressure.
Proficient in using computer technology.
