Registered Nurse (RN)

Job Summary

We are seeking a dedicated Registered Nurse (RN) to join our team for day shifts. The successful candidate will be responsible for effectively managing patient care and assisting members through their healthcare journey.

Key Responsibilities

• Conduct comprehensive assessments and evaluations of patients' healthcare needs.
• Develop and implement personalized care plans in collaboration with healthcare teams.
• Facilitate communication between patients and healthcare providers to ensure optimal care delivery.
• Provide education and support to patients and their families regarding treatment plans and health management.
• Monitor and document patient progress and health outcomes.
• Participate in case management and utilization management activities.

Qualifications

• Education: Bachelor's Degree in Nursing (BSN) is preferred.
• Licenses/Certifications:
  
  
  • Registered Nurse (RN) licensure in the state of practice is required.
  • Certified Case Manager (CCM) is preferred.
• Experience:
  
  
  • 5-10 years of experience in acute and ancillary care across multiple modalities is preferred.
  • Experience working in a health plan on utilization management and case management activities is preferred.

• Strong communication skills, capable of coaching patients through their healthcare journeys.
• Ability to work collaboratively in a team-oriented environment.
• Excellent organizational skills with attention to detail.
