Case Manager RN – Sanford, FL Employment Type: Full-Time Schedule: Days | Monday–Friday + Rotating Weekends Shift Options: 8:00 AM–4:30 PM, 9:00 AM–5:30 PM, 10:00 AM–6:30 PM, 11:00 AM–7:30 PM Pay: $36.96–$44.00/hour Weekend Differential: $1.50/hour Sign-On Bonus: Available on a case-by-case basis Relocation Assistance: Available on a case-by-case basis Overview Join a Level II Trauma Center as a Case Manager RN supporting patients across all inpatient service lines. This role coordinates discharge planning, care transitions, resource utilization, and patient advocacy while collaborating with physicians, nursing leadership, rehabilitation teams, and community providers.
