Behavioral Health Home Nurse Care ManagerThe Behavioral Health Home (BHH) Nurse Care Manager will work as part of an interdisciplinary team to provide community-based integrated physical and behavioral health services to individuals who may have complex and chronic mental illness, substance abuse, and medical comorbidity. Tests may include: HA1C, Lipids, WBC, Urine Pregnancy test, and Urine Drug ScreenAssesses and recommends laboratory testing based on an individual's medications, psychiatric and medical conditionsFacilitates the use of the Electronic Health Record (EHR) and other Health Information Technology (HIT) to link services, facilitate communication among team members and provide feedbackMeets regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinicIdentifies current chronic medical condition trendsAssesses barriers to care and implements solutionsIdentifies health care promotion and prevention servicesAttends Care Team MeetingsCompletes all required documentation completely, accurately, and in accordance with CNS policy.Assesses and monitors the health status of assigned individuals.Assists in the coordination and monitoring of medication for individualsSeeks out, links, and collaborates with primary and specialty care physicians, community health agencies, and other healthcare providers.Administers medicationsProvides education and monitoring for adverse reactions to medicationEstablishes a collaborative relationship with prescribers and clinic staffFacilitates weekly persons served doctor appointmentsQualifications:RequiredCurrent unrestricted RN licenseValid and unrestricted Driver's LicensePreferredBachelor of Science in Nursing (BSN) degreeOne year of experience (Full-Time) working with adults in a mental health settingAbout CNS Healthcare:CNS Healthcare (CNS) is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) with seven clinics and two clubhouses in Southeastern Michigan.
