Work Environment

The role of Family Nurse Practitioner will be primarily onsite at our hospital facility in Glenfield. Your workweek will average around 40 hours, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Furthermore, you may be required to engage in community events or health fairs on weekends, ensuring that we reach and serve the broader community effectively.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the medical needs of patients and families.
• Diagnose and manage both acute and chronic medical conditions, delivering preventive care and health promotion strategies tailored to individual requirements.
• Collaborate with community organizations to develop and facilitate health education programs, aimed at empowering individuals through knowledge and resources.
• Deliver culturally competent care while advocating for patients' rights, ensuring they have access to needed healthcare services.
• Coordinate referrals and follow-up care to promote continuity and quality of care for all patients within our system.

Qualifications

• Current certification as a Nurse Practitioner is required.
• An active nursing license specific to the state of New York.

Compensation and Benefits

We offer a highly competitive salary range of USD 180,000 to 200,000 per year, commensurate with experience and qualifications.
