Description

• Conduct patient evaluations in a range of settings to craft personalized treatment plans
• Deliver targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and rehab professionals to achieve optimal rehabilitation outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and exercise regimens tailored for home
• Maintain comprehensive documentation of evaluations, treatments, and progress in alignment with both facility standards and regulatory frameworks
• Engage in interdisciplinary care team meetings to ensure synchronized patient care

Education

• Doctorate in Physical Therapy (DPT) or a related accredited degree

Certifications

• State licensure as a PT / Physical Therapist
• Preferred but not required: CPR certification
• Specialized certifications (e.g., in geriatrics or neurology) are advantageous, though not essential
