Remote Central Intake Coordinator Be the link between hospitals, families, and outstanding care! Progressive Quality Care is looking for a remote central intake coordinator. We have gorgeous, hotel-like facilities, in which all of our residents are cared for in their own private rooms, with private bathrooms, including private showers. This is a remote position! The purpose of this position is to establish an effective system to gather information and communicate with the referral source before and upon admission to assure proper placement of the resident. Also, identify among the residents seeking admission those for which the facility can provide adequate care. Duties: Implement administrative policies and procedures to assure appropriate resident admission in compliance with federal, state and local requirements. Implement an organized and accurate record keeping system for resident admission. Gather sufficient resident information before admission to ensure facility is able to meet their clinical needs. Gather sufficient resident information including but not limited to pre-certification, LOCs, PASRRS to ensure a payor source prior to admission. Gather sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident. Ability to collect accurate data for all payer sources. Ability to interact with discharge planners in an effective, efficient manner. Ask appropriate and adequate resident information. Ability to analyze the information to determine if the facility can care for the resident adequately. Perform all duties as assigned in an effective manner. Follow admission and transfer policies and procedures. Keep accurate records of residents' information; keep all resident's information confidential. Work cooperatively with all facility department supervisors and administrative staff. Accept assigned duties in a cooperative manner. Ability to understand the facility computer system and the software used in the department. Come to work in neat, clean attire and consistently present an appropriate professional appearance. Come to work as scheduled and consistently demonstrate dependability and punctuality. Consistently work cooperatively with resident, resident's representative, facility staff, physicians, consultant and ancillary service providers. Qualifications: A minimum of a high school diploma; additional formal education preferred. Experience in long term care is required with sound knowledge base of the pre-admission process.
