Phaxis is partnering with an innovative Adult Day Health Care Program in New York City to hire a compassionate and experienced Registered Nurse (RN) for a temp-to-perm position with an ASAP start date.
Pay: Up to $50-52/hour (based on experience)
Location: Manhattan, NY
Schedule: Full-time
Position Type: Temp-to-Perm In this rewarding role, you'll work as part of a multidisciplinary team providing care and support to adults with chronic medical conditions and vision impairments. Responsibilities include: Conducting nursing assessments Developing and implementing treatment plans Coordinating healthcare services Providing individual and group health education Collaborating with an interdisciplinary care team
Qualifications:
- Current NYS Registered Nurse (RN) license
- Graduate of an accredited nursing program
- 35 years of recent experience in Med/Surg, Long-Term Care, or Community Health
- Strong assessment, communication, and documentation skills
- Proficiency in MS Word and computer documentation
- UAS-NY certification preferred
- Geriatric nursing experience is a plus
