Join an innovative, mission-driven organization redefining primary care for seniors through compassionate, relationship-centered, value-based medicine. We are seeking a dedicated Primary Care Physician (PCP) to join our growing team in Montgomery following a physician transition.

Our care model is designed around smaller patient panels, interdisciplinary collaboration, and proactive wellness-focused care for Medicare-eligible adults and underserved senior communities.

Physicians are empowered to build lasting relationships with patients while supported by an experienced clinical and operational team committed to improving outcomes, reducing barriers to care, and strengthening the communities we serve.

• Base Salary ranging from $250,000 for new graduates/residents up to $295,000 for experienced physicians
  
• Annual Incentive Bonus with achievable performance goals and recent physician payouts ranging from $45,000 to $85,000

• Smaller patient panels that allow more meaningful patient interactions

• On-site diagnostics including labs, EKG, and echocardiogram
  
• Collaborative interdisciplinary care team including nurses, MAs, case managers, social workers, and community outreach specialists
  
• Dedicated support for care coordination, patient navigation, and population health management
  
• Continuous professional development and training in value-based care
  

• MD or DO from an accredited medical school
  
• Board-certified or board-eligible in Internal Medicine or Family Medicine
  
• Active or eligible Alabama medical license
  
• Technologically savvy physician strongly preferred
  
• Experience in value-based care is a significant plus
