Responsibilities:
Provide direct patient care under the supervision of licensed nursing staff, including vital sign checks, bathing, dressing, and grooming
Assist with daily living activities, such as feeding, toileting, and ambulating
Monitor patient conditions and report changes to nursing staff
Maintain accurate and detailed records of patient care and treatment
Collaborate with interdisciplinary teams to develop and implement individualized care plans
