Nurse Case Manager
Wellmark
Wellmark is seeking a Case Management Nurse I to support members with chronic and complex health care needs. You will develop individualized care plans and collaborate with members, families, providers, and community resources to coordinate care.
The role requires an accredited nursing program, RN license in IA or SD, CCM within 24 months, and 4+ years of clinical experience. Remote eligible; strong communication and tech skills are essential.
