- Conduct comprehensive evaluations of patients in both inpatient and outpatient settings and tailor treatment plans accordingly
- Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Collaborate closely with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
- Educate patients and their families regarding therapeutic goals, discharge procedures, and home exercise regimens
- Maintain accurate documentation of evaluations, treatment plans, and patient progress in accordance with all facility and regulatory guidelines
- Engage actively in care team discussions and interdisciplinary meetings to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure required for Physical Therapist
- Preferred: CPR certification
- Specialized certifications (e.g., neurological or geriatric) are advantageous but not required
- Strongly preferred: Previous experience in a clinical setting
- Aptitude for caring for patients with multifaceted medical and mobility challenges
- Excellent communication and teamwork capabilities
- Comfortable utilizing digital charting and clinical documentation technologies
