LOCATION: American Canyon, CA

PAY RANGE: NP/PA Annual Salary: $126,147.84 to $154,169.60 (Based on a 32 hour work week)

FTE: 0.8 FTE (32 hours a week) (or 1.0 FTE) FYI, Hours of business is Monday to Friday 9am to 6pm

DLSE/FLSA STATUS: Exempt /Salaried position

About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW: The Family Nurse Practitioner or Physician Assistant will be responsible for providing quality primary health care to patients while working closely with other healthcare staff in a team approach.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

Education: Graduate of an accredited Master of Science in Nursing required. Current California Family Nurse Practitioner certification required. Must have or obtain current California medication furnishing license. Must have or obtain current DEA License. CPR Certification required

Education: Graduate of a Physician Assistant program from an accredited school required. Current California Physician Assistant License required. National Commission on the Certification of Physician Assistants Certificate required. Must have or obtain current California medication furnishing license. Must have or obtain current DEA License. Current Basic Life Support CPR certification required

Experience/Lived Experience: 3+ years of experience required. Experience in urgent care or primary care preferred. Experience in a community healthcare center preferred. Excellent interpersonal and customer service skills required, including trauma-informed de-escalation techniques.

Special Skills/Training: Strong verbal and written communication skills. Bilingual (English/Spanish or other threshold language) preferred. Must have strong time management skills, ability to prioritize, and work in a fast-paced environment. Ability to collaborate with mutual providers of treatment in primary care setting. Knowledge of Electronic Health Record and associated computer programs

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1. Provide the following health care services for patients: assessment/evaluation, diagnosis, treatment/prescribing, planning and education. Consult with staff physicians as needed

2. Review and manage patient information including labs, diagnostic images, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc.

3. Prescribe and administer medications and treatment regimens according to assessed medical conditions. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions

4. Refer patients to specialists and to relevant patient care services as appropriate

5. Prioritize patient care throughout the day

6. Provide acute triage over the phone or with walk in patients

7. Provide education and information to patients on various diseases, providing handouts as appropriate

8. Perform various in office procedures as directed

9. Document all appropriate information in electronic health record system

10. Respond to messages and relay information to patients and other external sources as needed

11. Direct and coordinate the patient care activities of nursing and support staff as appropriate

12. May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.

13. Assists in the coordination and integration of all health clinic outreach programs as directed

14. Participate in education of students

15. Perform other duties as assigned
