RN Supervisor of Service Delivery - Community and Network

Pay range: $82,899.44 - $107,371.73 annually

General Statement Of Duties:

Under the supervision of the Chief of Health Services and Integrated Care (CHSIC), provides supervision, coaching, mentoring and first line support to nurses, medical assistants, care coordinators and other providers on health care service delivery and nursing practice within Saginaw County Community Mental Health Authority (SCCMHA). Supervises and champions our RN-led, team-based, care management philosophy of care, to provide a health care safety net for all persons served by the SCCMHA. Supervises and ensures the provision of health education, health assessments, and general whole-person health care management to persons served with the goal of improving self-management and health outcomes. Supervises care coordination processes and ensures the efficient use of clinical workflows to achieve health outcomes specific to persons served.

This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid persons served in the recovery process.

Essential Duties And Responsibilities:

1. Supervise delivery of integrated health and physician services in community and network (liaison for parallel services), including but not limited to residential settings, community-based programs, nursing homes, nursing, and physician role in OBRA/PASSAR, and healthcare related agency trainings.

2. Serve as a nursing and health care consultant to the agency.

3. Perform agency performance evaluations for all assigned staff.

4. Supervises team huddles, holds regular staff meetings to promote communication and information sharing, other meetings as assigned, represents Physician and Health Services at meetings as assigned.

5. Ensures that staff are appropriately coached and mentored including recommendations for progressive discipline, if indicated, is applied equitably to all staff. Provide clinical mentoring, coaching, and training to care coordinators on day-to-day outpatient psychiatry services.

6. Supervises and provides nursing services using the nursing process and the Scope and Standard of Nursing Practice.

7. Supervises daily performance and productivity of the assigned staff to ensure duties are completed accurately, efficiently, and timely.

8. Monitor and review documentation of assigned staff to meet policy guidelines for evidence-based nursing care, and appropriate standard of care.

9. Interview and onboard new hires in assigned departments.

10. Liaison for the implementation of Health Information Technology (HIT) and leverages technology to improve outcomes.

11. Maintains, authors, updates, and implements policies and procedures while ensuring they comply with state, federal, and other applicable regulations.

12. Supervises all risk-mitigation evaluation strategies (REMS).

13. Serves as backup for assigned staff as needed, and maintains a caseload, as needed, at the direction of the CHSIC.

14. Participates as a member of the quarterly Leadership Team of SCCMHA Supervisors

15. Acts as liaison in the SCCMHA provider network regarding nursing scope of practice, roles, and internal system relationships.

16. Applies SCCMHA infection prevention and control policy and procedure throughout the agency.

17. Communicates the mission, vision, and core values of SCCMHA to staff while holding them accountable and implements these principles in all duties of this position.

Incidental Duties And Responsibilities:

1. May address community groups or organizations and represent SCCMHA in community venues such as community population health collaborative(s) at the direction of the CHSIC.

2. Attends in-service training, seminars, conferences, and workshops in order to sustain professional competency.

3. Reacts productively to change and manages other essential tasks as assigned.

Reporting Relationships:

Reports to: Chief of Health Services and Integrated Care

Supervises: Assigned Registered Nurses, CHWs, other department staff as assigned

Working Conditions/Environment:

Works in an office and community setting environment with the business pressures of dealing with time constraints and stress from long periods of computer use. Occasionally operates an automobile to attend appointments in the community and other sites, sometimes during bad weather. Close contact with the public and working with individuals with a developmental disability and/or mental illness who can be dangerous with possible injury to themselves and others.

Minimum Requirements:

Education:

Associate's degree in Nursing required. Bachelor's degree in Nursing preferred. Advanced degree preferred

Experience:

Nursing supervisor, mentoring and/or coaching experience required. Minimum 5 years of service delivery within the Michigan community mental health system required. Medicaid system, psychiatric outpatient services, and/or specialized residential nursing services preferred. Graduate of an accredited school of nursing with at least 2-3 years of clinical experience in a medical setting preferred. Substance use disorder experience preferred.

Licenses and Certifications:

Valid Michigan Driver's license with a good driving record. A licensed registered nurse (RN).

Knowledge, Skills and Abilities:

1. Knowledge of scope of the registered nurse, licensed practical nurse, medical assistants, Community Health Workers (CHW), Peers, and other service providers within the Michigan CMH system.

2. Knowledge of and ability to apply the standards of nursing practice.

3. Knowledge of and appropriate application of the nursing process.

4. Knowledge and ability to apply the methods, practices, protocols which are evidence-based practices and quality improvement measures.

5. Promotes a work environment that supports positive work relationships and high-performance teams.

6. Ability to implement methods and principles of patient and staff education.

7. Identify and appropriately manage ethical and cultural aspects of situations when making decisions.

8. Strong nursing skills are required and the ability to function as a member of an interdisciplinary team is critical.

9. Effective oral and written communication skills, minimum at college level. Communicates ideas and information verbally and in writing and the ability to prepare documents and presentations in multiple media. The ability to tailor communication to the reader in format and complexity.

10. Computer competency, including EHR, Microsoft 365 tools (independently proficient at using Word, Excel, OneNote, Teams, Outlook, and other tools as needed).

11. Ability to exercise independent and mature judgment.

Physical/Mental Requirements:

1. Hearing acuity to converse in person and on telephone.

2. Visual Acuity to assess person served, read, and proofread documents, and use computers.

3. Ability to walk, stand or sit for extended periods of time.

4. Manual dexterity to write and to operate standard office equipment (PC, Keyboard, Copy Machine, Fax Machine, etc.)

5. Ability to lift and carry files and supplies at least 20 pounds.

6. Strong interpersonal skills to interact with leadership, employees, persons served, and the general public.

7. Analytical skills necessary to conduct research, analyze, and interpret complex data and identify and solve problems by proposing courses of action.

8. Ability to plan short and long range and to manage and schedule time.

9. Ability to handle stress in meeting deadlines and dealing with large numbers of employees and/or persons served.
