The Care Manager plays a vital role in coordinating comprehensive care for patients within hospital, community/outpatient, and medical office settings. This position focuses on patient assessment, advocacy, and collaboration with interdisciplinary teams to ensure seamless delivery of health services and support for patients and their families.

Responsibilities

• Coordinate care plans across various healthcare settings
• Conduct thorough patient assessments to identify needs and resources
• Advocate on behalf of clients to ensure quality and timely care
• Collaborate with medical providers, social services, mental health, and allied health professionals
• Maintain detailed and accurate documentation of care activities
• Provide support and education to patients and their families
• Manage follow-up appointments and care transitions
• Occasionally travel locally to meet with patients or collaborate with teams

Preferred Qualifications

• 2+ years’ experience in care management
• Strong skills in care coordination and patient assessment
• Excellent communication, problem solving, and time management abilities
• Empathy and interpersonal skills to provide compassionate support
• Proficient in documentation and maintaining thorough records
