RN Care Navigator: Triage, Education & Coordination
IHA
IHA is seeking a dedicated Registered Nurse to provide professional nursing care in an outpatient setting. The RN will triage patients by phone and during visits, ensuring timely access to the appropriate level of care and reducing care fragmentation. Collaborating with physicians, care managers, and the broader care team, the RN will support transitions of care, document findings, and perform medication reconciliation. HIPAA compliance and patient safety remain central to daily duties. #J-18808-Ljbffr IHA
