Nurse Practitioner
Carease Health is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals in Jackson, TN. As a Nurse Practitioner with our company, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities while enjoying a hybrid work arrangement that combines on-site Weekend visits with remote work from home.
About the Role:
Incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program, we prioritize proactive measures for enhanced physical health and well-being. As a Nurse Practitioner with Carease Health, you will work closely with our experienced clinical team to provide patient-centered care that minimizes hospitalizations and elevates patient care standards.
Responsibilities:
Deliver high-quality preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
Build strong relationships with facility staff and maintain a patient-centered approach
Collaborate with our clinical team to provide comprehensive care to patients
Stay up-to-date with the latest medical research and guidelines to ensure that patients receive the best possible care
Requirements:
Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
