Minimum one year's experience. Current state license as a Registered Physical Therapist / PT or be in the process of obtaining one. Current CPR certification. Duties and Responsibilities The Physical Therapist / PT is responsible for the assessment of referred patients requiring rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation. The PT will establish and administer a treatment program; set realistic, achievable goals; document the patient's condition; and educate patients and families in appropriate physical therapy methods. Develop a plan of care for each physical therapy patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient.
