Work Environment This position requires an onsite presence as you will be providing home care services directly to clients' residences. Key Responsibilities Conduct comprehensive evaluations for clients, particularly children, assessing physical capabilities and therapeutic needs. Develop, implement, and adjust individual treatment plans based on thorough assessments and ongoing evaluations, ensuring alignment with patient goals. Continuously monitor and document patient progress, adjusting therapeutic strategies as needed to enhance effectiveness. Utilize our electronic health record system to maintain accurate, detailed documentation, including SOAP notes and care plans, within specified timeframes. Provide education and guidance to patients and their families on the rehabilitation process, addressing potential challenges and supporting their journey towards recovery. Achieve established utilization goals and contribute to fulfilling organizational objectives through collaborative efforts. Engage in multidisciplinary teamwork, coordinating with other therapists and professionals to provide holistic care for clients receiving multiple services. Embody and promote the values and culture of Motion Menders Therapy, acting as an ambassador for our mission and vision. Patient Population / Client Focus Your patient population will primarily include children, as well as adults, with varying physical therapy needs stemming from congenital issues, developmental delays, sports injuries, and post-operative rehabilitation. You will implement therapies tailored to each individual's unique circumstances, ensuring inclusivity across age, race, and ethnicity. This focus on diverse patient backgrounds underscores our commitment to equitable healthcare delivery. Qualifications A Bachelor's degree in Physical Therapy; a Master's or Doctor of Physical Therapy (DPT) is strongly preferred. Active and unrestricted licensure as a Physical Therapist in the State of New Jersey. Willingness to perform therapy evaluations and sessions both in-person and via telehealth, where clinically appropriate, adapting to the current healthcare landscape. Flexibility and a strong desire to work with a diverse patient demographic, including a range of ages, cultural backgrounds, and medical conditions. Excellent collaborative skills and a commitment to customer service excellence. Preferred Experience Experience in pediatric physical therapy is highly valued, especially in home care settings. Demonstrated ability to work effectively in a multidisciplinary environment, collaborating with other therapy disciplines. Proven track record of meeting performance goals and contributing to organizational success. Compensation and Benefits The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience.
