The Case Manager assists the physicians and staff to determine the appropriate level of care and post-acute care needs. This position reports to the Director, Case Management and Social Work and operates within established organizational and departmental policies and procedures.**Qualifications****Education*** Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferredCan this role accept experience in lieu of a degree?
