About the Role:

As a Social Worker (Nursing Home) , you will play a critical role in ensuring the social, emotional, and psychological well-being of our patients and residents. You will work closely with our interdisciplinary team to assess, plan, and implement interventions to address the unique needs of each individual. Your expertise will help us provide person-centered care that promotes dignity, respect, and autonomy.

Responsibilities:

• Conduct social work assessments and develop comprehensive plans to address the needs of patients and residents

• Identify and address social, emotional, and psychological issues that may impact a patient's or resident's care

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Provide counseling, support, and education to patients, residents, and their families

• Document all interactions, assessments, and care plans in accordance with facility policies and regulations

• Maintain accurate and up-to-date records of patient and resident interactions and care plans

• Participate in continuous quality improvement initiatives to improve patient and resident outcomes

Requirements:

• Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program

• Licensure or certification as a Social Worker in the state of Pennsylvania

• Minimum of [2] years of experience in a long-term care setting
