Licensed Practical Nurse (LPN) - DCF Program

Licensed Practical Nurse (LPN) - DCF Program Location: Hampton, NJ (Hunterdon County) Licensed Practical Nurse (LPN) Purpose To provide nursing and direct care to individuals residing in homes or receiving services provided by Allies, Inc. The clients we serve have varying medical needs that must be met by compassionate nurses with the skills to care for them. LPNs at Allies ensure that clients have proper health maintenance and medical treatments through their own care, and through supervision and training of the direct care staff. Requirements Graduate of accredited Nursing School; LPN Licensed in the State of New Jersey Valid Unrestricted Driver's License with good driving record One year of experience working with ID/DD youth. The ability to lift and transfer at least 50 pounds. The ability to sit, bend, squat, reach, be agile, and physically pursue individuals on foot and stand for several hours at a time Basic Computer Literacy Must be able to read, write and verbally communicate in English and direct/respond to individual's needs. Required Skills The ability to exercise good judgment and use common sense. The ability to remain calm in crisis situations. The ability to implement trauma-informed care. The ability to interact with a wide range of people with respect. The ability to approach daily work from a person-centered viewpoint. Responsibilities and Duties Provide direct nursing care effectively by ensuring the general well-being of each youth while promoting a trauma informed approach to health care. Administers medication in lieu of milieu staff and completes documentation of same. Assess the physical condition of the children in the program and integrate findings into the child's treatment plan. Provides assistance with activities of daily living skills (ADLs). Accompanies youth to and from medical appointments and activities. Educates youth and staff regarding health, hygiene, nutrition, body image and general well-being. Participate in initial treatment and crisis plan consultation, youth meetings, new admissions and ongoing. Implement quality assurance program. Implements medically prescribed diets and exercise programs.Assists the Registered Nurse in training of Support Staff in the areas of health maintenance, medications (including possible side effects), and medical treatments. Encourages and ensures that individuals maintain a clean, safe, and comfortable environment. Accurately completes all in-house documentation including but not limited to Vehicle Log, behavioral tracking, Antecedent Behavior Consequence (ABC) forms, Unusual Incident Reports (UIRs), Critical Information Log, Medication Administration Reports (MAR), Progress notes and Monthly reports as assigned. Successfully completes required residential training, including CPR and First Aid; Medication Administration; Preventing Abuse, Neglect and Exploitation; Shifting Expectations, Life Experience and Services; Life Threatening Emergencies (Danielle's Law); New Hire Orientation; Crisis Management; Corporate Compliance; General Behavior; In-House Orientation; and any required specialized trainings. Completes annual training, including role specific trainings relative to Accreditation and Continuous Quality Improvement. Attends Treatment Team meetings and works with the Treatment Team to build self-esteem and social capital. Operates all types of agency vehicles, in any condition, at any point during shift. Other duties as assigned.
