divh2Registered Nurse/h2pThe registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities./ppPatient Care:/pulliCompletes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)./liliRegularly re-evaluates patient nursing needs./liliInitiates the plan of care and makes necessary revisions as patient status and needs change./liliUses health assessment data to determine nursing diagnosis./liliDevelops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process./liliInitiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician./liliCounsels the patient and family in meeting nursing and related needs./liliProvides health care instructions to the patient as appropriate per assessment and plan of care./liliIdentifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient./liliActs as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload./lipCommunication:/pulliPrepares clinical notes and updates the primary physician when necessary and at least every 60 days./liliCommunicates with the physician regarding the patients needs and reports any changes in the patients condition; obtains/receives physicians orders as required./liliCommunicates with community health-related persons to coordinate the care plan./li/ulpAdditional Duties:/pulliParticipates in on-call duties as defined by the on-call policy./liliEnsures that arrangements for equipment and other necessary items and services are available./liliInstructs, supervises and evaluates home health aide care provided every two (2) weeks./li/ulpRequirements:/pulli1 year of Home Heath experience - REQUIRED/liliGraduate of an accredited school of nursing with 1 year of recent acute care experience - REQUIRED/liliMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - REQUIRED/liliGood typing skills - REQUIRED/liliWound care, wound vac, catheter, acute care experience - REQUIRED/li/ulpCERTIFICATION REQUIREMENTS:/pulliCPR/li/ulpSTATE LICENSE REQUIREMENTS:/pulliCalifornia/li/ulpShift Notes: 40 hours scheduled/Must be able to work a Tuesday-Saturday or Wednesday-Sunday schedule Shift: Day/ *8 Hours Duration: 13 weeks Benefits Great pay Full benefits Local and out-of-state travel opportunities (hundreds of clients Nationwide) Backing by a team that has been in the industry for over 30 years/p/ul/div
