Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking an energetic, results-driven Marketer / Hospital Liaison to join our growing team. This is an outstanding opportunity for a motivated healthcare marketing professional who enjoys building strong referral relationships, increasing census, and representing a premier skilled nursing and rehabilitation center.

As the Hospital Liaison, you will serve as the primary point of contact for hospitals, physicians, case managers, discharge planners, and community partners, helping ensure a smooth transition of patients into our facility while expanding referral opportunities.

Responsibilities

• Develop and strengthen relationships with hospitals, physicians, case managers, discharge planners, assisted living communities, and other healthcare partners.

• Generate new referral sources while maintaining and growing existing referral relationships.

• Conduct daily in-person visits to hospitals, physician offices, and community partners throughout the assigned territory.

• Educate referral sources on the facility's clinical programs, rehabilitation services, and specialty offerings.

• Coordinate with the Admissions Department to ensure timely evaluation and admission of referrals.

• Track referral activity, conversion rates, and marketing performance using CRM tools and reporting systems.

• Attend networking events, healthcare meetings, and community outreach programs to increase facility visibility.

• Collaborate with facility leadership to develop and execute strategic marketing initiatives that drive census growth.

• Maintain a strong understanding of Medicare, managed care, and post-acute care services.

• Provide exceptional customer service to referral partners, residents, and families throughout the admissions process.

• Stay informed of market trends and identify opportunities to expand referral partnerships.

Qualifications

• Previous experience as a Hospital Liaison, Clinical Liaison, Healthcare Marketer, Admissions Director, or Business Development Representative preferred.

• Skilled Nursing Facility, Acute Care Hospital, Rehabilitation, Home Health, Hospice, or Healthcare Sales experience strongly preferred.

• Proven ability to build relationships and consistently meet or exceed census and referral goals.

• Excellent communication, presentation, and interpersonal skills.

• Strong organizational skills with the ability to manage multiple accounts and priorities.

• Self-motivated, professional, and results-oriented.

• Valid driver's license and reliable transportation for local travel.

Job Type: Full-Time
Schedule: Monday–Friday, with flexibility for networking events and referral partner meetings as needed
