Compassionate Hospice RN Case Manager

Gentiva's Heartland Hospice is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate care in patients' homes across Davenport, IA. You will work with an interdisciplinary team to implement plans of care and provide education and bereavement support. The position requires an active Iowa RN license and at least one year of RN experience in hospice, home health, or acute care. Strong communication and organizational skills, and reliable transportation, are essential. #J-18808-Ljbffr Gentiva
