Job Description:

We are seeking a highly experienced RN Case Manager with acute inpatient case management and discharge planning experience for our facility in Jackson, Mississippi. The candidate will need to quickly adapt and effectively manage a caseload across all inpatient units.

Responsibilities:

• Manage a caseload of 15-20 patients per day.
• Work collaboratively with the Social Work team and other departments for discharge planning.
• Utilize Epic EMR for charting and documentation.
• Participate in department-specific orientation over a two-week period.
• Consult with RT and Pharmacy as needed for discharge planning.

Requirements:

• Minimum of 3 years of acute inpatient case management experience, with at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center).
• Previous travel experience in a level 1 facility.
• Strong computer skills with experience using Epic EMR.
• Nursing degree preferred RN, BSN; will consider ADN. BLS certification through AHA is required, CCM preferred but not mandatory.
• Recent acute discharge planning experience within the last two years.
