Job Summary: Provides spiritual support and general counseling to patients/families and bereaved families. Performs memorial services and funerals for patients. Serves as a liaison between community clergy and Hope Hospice. Promotes end-of-life care within and through area churches.

Qualifications:

• Bachelor's degree in Theology or Divinity or related field such as Pastoral Counseling; Master's degree preferred.
• Ordination or certification as lay minister or chaplain from a recognized religious organization, optional
• Ability to perform all duties and responsibilities of the job description.
• Suitable personal appearance, behavior, attitude, and integrity.
• Able to problem solve, make decisions, set priorities and document using electronic technologies in an effective and timely manner.
• Computer and software competence.

Duties and Responsibilities:

A. Spiritual Care Plan

• Assesses the spiritual needs of the patient and/or family through personal interview and consultation with other interdisciplinary team members. Develops an individual spiritual care plan which includes collaboration with local clergy whenever possible.
• Provides spiritual support as needed and preferred by the patient/family/significant other.
• Offers patients/families regardless of philosophy or religious perspective, the opportunities to discuss and share their thoughts, feelings, beliefs, and values to enhance their spiritual understanding of life & death.
• Evaluates progress toward meeting patient/family goals related to unresolved spiritual issues and patient/family response to care. Adjust care plan to meet patient/family needs/goals.
• Provides support and counseling for patients and family in support of other needs identified as a part of the care process.
• Works in cooperation with social work to assure a continuum of spiritual and psychosocial service is achieved.

B. Team/Communication

• As a member of the Inter-Disciplinary Team (IDT) evaluates patient/family response to care, including attendance at IDT conferences when the care plan is reviewed.
• Assists members of the IDT to understand the significant spiritual concerns and/or religious beliefs impacting the patient/family response to end of life.
• Provides on-call chaplain services as arranged and responds appropriately.
• Documents all contacts with patient/family at the time of care or written 24 hours of contact.

C. Community Liaison

• Integrates existing spiritual support systems into the plan of care. Solicits support from specific faith systems when requested by the patient/family.
• Serves as a liaison to and a resource for community clergy in dealing with Hospice patients end of life and issues of death and dying.
• Provides educational programs for community clergy and lay representatives.
• Actively develops relationships with community clergy and lay members of area churches.

D. Religious Services & Rituals/Bereavement

• Conducts religious services and rituals for patients/families as requested.
• Performs occasional liturgical assignments such as the annual Memorial Service and monthly and special event activities for staff.
• Works closely with members of the IDT and the Director of Bereavement to provide optimal support for patients/families.

E. General

• Supports philosophy and objectives of Hope Hospice.
• Promotes a team approach through cooperation, mutual respect, flexibility, and dependability.
• Participates in appropriate meetings and activities, including Performance Improvement.
• Exhibits good stewardship of resources, including self-care.
• Seeks personal growth and contributes to an atmosphere of excellence, respect, and caring.
• Reads materials and attends in-services, seminars, or conventions as needed for professional growth.
• Maintains ordination or licensure in good standing within own denomination.
• Maintains current knowledge of agency policies and procedures and accreditation and governmental regulations affecting area of expertise.
• Performs other duties as assigned.

Working Conditions:
Is required to operate a motor vehicle in order to drive to and from assignments in patient homes and nursing facilities. Duties include climbing stairs of varying sizes, types, and degrees that may be slick or in disrepair; negotiating ramps of varying sizes, types, and degrees that may be slick or in disrepair; and dealing with various issues related to the yard and street; and dealing with other circumstances both outside and inside the patients place of residence. Must be able to
perform under a wide variety of conditions and circumstances in the home. Potential for exposure to blood and body fluids and infectious material. Must be able to communicate in writing and verbally, both in person and by telephone. Normal office environment; moderate amount of standing, walking, stooping, sitting; May be required to repeatedly lift, carry, push and pull greater than 50 pounds, and carry agency provided electronic equipment for completing the Electronic Medical Record (EMR) in the home.

Travel
There is no anticipated overnight travel for this position. Local day travel is required.
