Physical Therapist (PT) Specialty Home Health
Schedule: 8-hour day shifts only
Assignment Details: Guaranteed hours: up to 6 patient visits/day; may call off up to one shift per pay period
Requirements: License: required, with acceptance of pending licenses Experience: 1 year of skilled home health experience Certifications: BCLS EMR: experience with OASIS documentation expected
Roles & Responsibilities: Evaluate and assess patient needs to develop appropriate treatment plans Implement home exercise programs and provide patient/family education Facilitate post-operative rehabilitation for orthopedic and neurological conditions Monitor and document patient progress and adjust treatment plans as necessary Utilize safe body mechanics and mobility techniques during patient interactions
Compliance / Notes: Dress Code: business casual attire (scrubs allowed but not required) Parking: free on site Communication: clinician responsible for own communication device if not provided
