GENERAL STATEMENT OF DUTIES

The Advanced Practice Provider provides comprehensive, evidence-based care to pediatric patients within the scope of professional licensure, certification, clinical privileges, applicable practice arrangements, and organizational policies. This position collaborates with physicians, residents, nurses, dietitians, diabetes educators, and other healthcare professionals to support accurate diagnosis, effective treatment, patient education, and continuity of care.

 

ESSENTIAL FUNCTIONS

· Conduct comprehensive health assessments, including medical histories, physical examinations, preventive screenings, and evaluation of relevant clinical information. 

· Order and interpret laboratory tests, imaging studies, and other diagnostic procedures necessary to evaluate patient conditions and treatment response.

· Develop, implement, and periodically update individualized treatment plans based on clinical findings, patient needs, and established standards of care.

· Prescribe and monitor medications and other treatments in accordance with applicable licensure, prescriptive authority, practice arrangements, clinical privileges, and organizational policies.

· Monitor patient progress, evaluate treatment effectiveness, modify care plans as clinically indicated, and coordinate specialty consultation or referral when necessary.

· Educate patients and families regarding diagnoses, medications, treatment plans, nutrition, exercise, preventive care, health maintenance, diabetes self-management, glucose monitoring, medication administration, and applicable diabetes-related technology.

· Collaborate with physicians, residents, primary care providers, specialists, dietitians, diabetes educators, nurses, and other healthcare professionals to develop and coordinate comprehensive patient-care plans.

· Maintain accurate, timely, and complete documentation of assessments, treatment plans, prescriptions, patient progress, and care coordination in the electronic medical record.

· Follow applicable professional standards, clinical protocols, privacy requirements, practice arrangements, and organizational policies.

· Participate in chart review, quality improvement, population health, patient-safety activities, clinical education, departmental meetings, and assigned organizational committees.

· Maintain required licensure, certification, clinical privileges, continuing education, and professional competencies.

· Perform other duties as assigned.

REQUIRED EDUCATION & EXPERIENCE

· Graduate degree from an accredited Nurse Practitioner or Physician Assistant educational program required.

· Must satisfy one of the following professional qualification pathways:

· Nurse Practitioner: Current, unrestricted Michigan registered nurse license and Nurse Practitioner specialty certification, with current national board certification appropriate to the assigned patient population.

· Physician Assistant: Current, unrestricted Michigan Physician Assistant license and current certification through the National Commission on Certification of Physician Assistants.

· Current prescriptive authority and applicable federal and Michigan controlled substance registrations required or must be obtained within an established timeframe following hire.

· Must maintain all practice agreements, delegation arrangements, or other professional practice requirements applicable to the provider’s license and assigned responsibilities.

· Current Basic Life Support certification required.

· Clinical experience in endocrinology, diabetes management, internal medicine, or a related specialty preferred.

· Experience with electronic medical records and applicable clinical technology preferred.

· Must meet organizational credentialing, privileging, and professional liability coverage requirements.

· Strong clinical assessment, communication, patient-education, and interpersonal skills, with the ability to collaborate effectively with patients and interdisciplinary healthcare teams.
