Registered Nurse (RN) - Case Management

RN Care Manager

Schedule: Variable shifts: 8:30 am - 4:30 pm or four 10-hour shifts, 7:30 am - 5:00 pm. No call, weekend and holiday rotation required.

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Associate or Bachelor degree in nursing
• 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management and/or utilization review
• Utilization review and discharge planning experience
• Active Basic Life Support (BLS) certification
• Certification by a nationally recognized case management or utilization review organization preferred
• Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred

Roles & Responsibilities:

• Conduct assessments of patient needs and identify required resources for patient transitions
• Collaborate and lead interdisciplinary team meetings for care planning
• Educate team members and ensure excellent communication within the healthcare team
• Coordinate with community organizations for resources such as durable medical equipment and transportation
• Participate in managing inpatient lengths of stay and overall patient care quality measures

Timecards must be filled out correctly to avoid rejection and processing issues.
